Preplanning Checklist

The following checklist will help you create your personalized plan.

Ways to Plan Ahead:

 

General

  • Gather personal information for the paperwork and obituary.
  • Choose a church or charity to direct donations to, if desired.
  • Make note of any instructions for the services and final disposition.

Funeral Service

  • Choose burial or cremation.
  • Select a casket or cremation container.
  • Choose locations and types of services.
  • Choose floral arrangements.
  • Select which photographs and memorabilia you’d like to display.
  • Consider any customs, traditions, or religious rites that are important to you and your family.
  • Choose the clothing to be dressed in.
  • Select music and hymns, if desired.

Participants

  • Choose a Pastor, clergy member or officiator to preside over the service.
  • Select any Singer(s) and Musician(s) whose talents you’d like to employ.
  • Select pallbearers. (Traditionally there are six Pallbearers)
  • Choose friends or family members to perform the eulogy, read scripture, or prepare a speech.

Final Disposition

  • Choose a cemetery.
  • Select a burial plot or mausoleum.
  • Select a monument or grave marker and inscription. (See Monument Tab)
  • Decide who will keep your urn.
  • Decide who will scatter your ashes and where.

Personal Information You Will Need For The Certificate

  • Your name as it appears on your birth certificate
  • Your social security number
  • Your date of birth
  • Your birthplace (City & State)
  • Your address (Inside or Outside of City Limits?)
  • Did you serve in the military?
  • Your father’s name as it appears on his birth certificate
  • Your mother’s name as it appears on her birth certificate
  • Your place of disposition (Cemetery or other)
  • Your level of education
  • Your race
  • Your Occupation For the Majority of Your Life
  • Your desired Informant's Name, Relationship and Address (Typically your legal Next Of Kin)

Affiliations

  • Church Membership
  • Fraternal organizations
  • Service organizations
  • Local memberships
  • Any special recognitions you have

Military Service Information

  • Military service branch and copy of DD214 (Final Discharge Paperwork)
  • Branch rank
  • Name(s) of war/conflict(s)
  • Serial number
